Biomechanical Evaluation

Dr. Patish examines your feet at rest and in motion. Gait, alignment, range of motion, and pressure pattern are all assessed before any decisions are made.

The Casting Appointment

A precise impression of each foot is taken in the office. The casting captures the contour of your foot in a neutral position so the lab can build to that exact shape. Most people find it quick and comfortable.

Prescription & Lab Crafting

Dr. Patish writes the orthotic prescription based on the clinical findings and your daily activities. The impression and prescription go to a specialized lab, which fabricates the orthotic to clinical specifications.

Fitting & Follow-Up

At your fitting visit we confirm comfort and adjust as needed. A break-in schedule and follow-up plan support the transition so you are not guessing your way through it.

Coverage for custom orthotics depends on your specific plan and the clinical reason they are needed. We verify benefits before any services are committed to, so there are no surprises.

  • Commercial plans: Many cover medically necessary functional orthotics when prescribed for a documented condition. Coverage varies; some plans classify orthotics as a non-covered benefit.
  • Medicare: Therapeutic shoes and inserts are covered for qualifying patients with diabetes who meet specific clinical criteria. Other custom orthotic prescriptions are generally not covered by Medicare.
  • HMO referrals: Most HMO plans require a referral from your primary care doctor before podiatry visits. Call our office and we will help you start that process.
  • Self-pay: If your plan does not cover orthotics, we offer competitive self-pay pricing. CareCredit financing is available.

When Orthotics Aren't the Answer

Custom orthotics aren't a cure-all. For some patients, the right answer is rehab exercises, footwear changes, an injection, shockwave therapy, or in select cases a referral to a colleague for surgical evaluation. Dr. Patish will tell you when orthotics aren't the right answer for your situation, and recommend what is. Getting the right answer matters more than getting an answer.

What is the difference between custom orthotics and store-bought insoles?

Store-bought insoles are mass-produced for an average foot shape and primarily provide cushioning. Custom functional orthotics are made from a precise impression of your feet, designed to address your specific biomechanical issues, and built to clinical specifications. They are intended to correct function, not just pad the foot.

How are custom orthotics made?

Dr. Patish performs a biomechanical evaluation and a gait assessment, then takes an impression of each foot. That impression and the clinical prescription are sent to a specialized lab, which fabricates the orthotic to your specifications. The finished orthotics are checked at a fitting visit.

Will custom orthotics fit in my shoes?

Most custom orthotics fit in standard athletic shoes, work boots, and dress shoes with removable insoles. During your consultation we discuss the shoes you wear most often so the orthotic is designed to work with your real-world footwear.

How long do custom orthotics last?

Lifespan varies by patient weight, activity level, and wear patterns. Many patients use the same pair for several years with periodic refurbishing of the top cover. We will let you know when yours show signs of needing replacement.
